Submission Requirements:
Completed application with all of the following supporting documents:
• Less than 3 years or loss runs require a new venture profile
• Current Motor Vehicle Records for ALL drivers.
• Proof of CDL experience. Experience shown on MVR - Copy of the CDL
• Verification of expiring coverage
• Current valued loss runs with 3 years of historical data ( or equal to the number of years of prior coverage)
Eligibility for Risk:
• Power units 20 years or newer
• Trailers 25 years or newer
Ineligible Risks:
• Hazardous Materials
- Radioactive Material
- Volatile gasses
- Chemicals
• Double or Triple tractor–trailer trucks
• Public Transportation
• Coal
• Hay
• Logging/ Pulpwood
• Livestock
• Swinging Meat
• Mix-in Transit
• Mobile Homes
• Trash Haulers
Driver Requirements:
• Must hold a valid CDL
• Minimum of 1 year of CDL experience
* Will accept less than a year of CDL experience on case by case basis with a surcharge
• Bi-National Drivers can be considered for coverage with a surcharge
• Acceptable ages 21-70 years old
• Minimum driving age 21
* MVR must be clean and have 1 year of CDL experience
* Subject to a surcharge
• Drivers between 65– 70:
* MVR must be clean and have an acceptable long form medical certificate
* Subject to a surcharge
• MVR’s are reviewed on a 36 month basis for minor violations.
• Up to 3 minor violations allowed
* Subject to surcharge
• Major Violations allowable with a surcharge. Major violations: reckless driving, DUI, driving with suspended license, hit and run, evading a policy officer, etc.
• Maximum of 1 at fault accident in the past 5 years
* Subject to surcharge
