Available for New Ventures that can verify 1 or more years of experience delivery of hazmat.
Existing operations with 1 or more years in business.
Submission Requirements:
- Complete Application - email for specific
- 2 or more units
- 1 to 5 years of loss runs depending on age of operation - not older than 60 days
- Driver list with dates of hire, DOB, state license - excel format
- MVRs not older than 60 days
- Drivers - 2 or more years CDL at time of expiration/policy placement
- Equipment list with VINS, ages, & values - excel format
- 4 quarters IFTA for interstate
- Mileage statement for intrastate
Exclusions
- No international drivers
Cameras - are required.
