- Completed ACORD Application; include effective date, description of operations & exposures/premium basis (EG: sales, payroll, sq ft area/# of units, etc.)
- Currently Valued 5-Year Carrier Loss Runs
- Copies of primary policy decs or quotes/binders; include pricing/terms/conditions
- Underlying Policy’s Carrier must have an A.M Best Rating of B+ or better
