- Eligible Risks: Paid and combination (paid/volunteer/paid-on-call) Ambulance & NEMT operations. Para-transit transport is excluded.
- Minimum Premium: $20,000
- Required Forms: Acord 125 & Acord 130
- Mandatory SHIELD Telephonic Loss Control Assessment Prior to Quoting
Eligible States:
- Not Available in: AK, HI, NY, OR, UT, VT and Monopolistic.
- Limited Availability: Fire risks are not available in CA, NJ and PA.
- State-Specific Availability: Coverage for Fire and Ambulance operations in AR, LA, MO, OK, and SC may require additional review. Contact us to discuss potential placement options.
