Back Declination letter for florida wc

Can anyone send me a sample or declination letter for the FWCJU. It seen that none of mine are good. Been working in this first and last application. If anyone can get me that declined letter for a general contractor . They dont want any peo or leasing . my email millieb699@gmail.com
