    looking for workers comp carrier for a wholesale produce.  has 2 drivers and several clericals.  customer had policy but lapsed 2 months ago.

    If you could email or fax your old Declarations page to include the name, address and contact info of the business and a breakdown of payroll showing the amount of each class code of the employees I'll put together a quote.
